Dayton Chautauqua Center Applications

For applications to 2015 Chautauqua courses offered by the University of Dayton Chautauqua Center, we offer applicants the following options:

Option 1

To make an electronic application, make a copy of the application form below, fill it out electronically in Notepad or Word, and email it to the Dayton Chautauqua Center. Then mail a check* for the application fee.

Option 2

To make regular mail application, mail a check* for the application fee along with the completed application to the address below. Then send an email to say that it is coming.

*Application Fee

More than one course can be on the application form. The application fee is $100 for each course. If an appointment is not made, the fee is completely refundable without question. After appointment, the fee is no longer refundable. To send the fee, mail a check by regular mail made to "Chautauqua Program" to the following address:
