Dayton Center Applications

On Friday February 15 the Dayton Center web site experienced a server failure. On a new server the Dayton Chautauqua Center application process is not functioning correctly. While information technology people are working on it, we offer applicants the following options:

Option 1

To make application, mail a check* for the application fee along with the following five pieces of information: name, course, mailing address, phone number, and email address to the address below. Then send an email to say that it is coming.

Option 2

Make a copy of the application form below, fill it out electronically in Notepad or Word and email it to the Dayton Center. Then mail a check* for the application fee.

*Application Fee

The application fee is $50 per course until Feb. 29, then it is $100. To send the fee, mail a check made to "Chautauqua Program" to the following address:
