The Office of Human Resources
has worked to create several performance management tools for
supervisors to use in their effort to assess employee performance.
Supervisors now have the choice of using one of three options, including
a goal based option, a competency based option, or a narrative format
for providing ongoing feedback to employees.
Forms for each option are
provided below. Please contact your employee relations manager for more
information or if you would like to arrange for training on use of these
