FAQ's on How to Complete the PAF
INSTRUCTIONS for completing and routing the PAF:

1.    Download the current version of the form.  Right click on the link and select "Save Target As."  This will give you the opportunity to save to your local computer and open the form from that location.  There are tables behind selected fields for the Zip +4, Building, Department, and Division.  It is important that you access the form from the website to insure you have the most up-to-date version.  The form is black and white and should adjust to print on one page with the printer used in your office.

2.    Complete the form.  You may save the completed form on your computer, however, it will not be accepted if you send it via e-mail to Human Resources. 

3.    Print the form and acquire appropriate signatures.  Feel free to photocopy before sending to HR.

4.    Send only the original signed copy to the office of Human Resources at Zip +1614 or Student Employment at Zip +1306 if the action is for an Undergrad or Grad Student.

5.    Human Resources will make a copy for Payroll and a confirmation copy to the initiating department after the action has been processed.
