All staff members must complete the following documents in the office of human resources, St. Mary's Hall Room 118.

DOCUMENTATION - The following must be completed and placed on file when beginning employment:

  1. Application for Employment

  2. Federal (Form W-4) and a state withholding exemption certificate

  3. Personal Statistical Data Sheet - Staff or Faculty

  4. I-9 Form required by Immigration Reform and Control Act

  5. Handbook Receipt

  6. Employment Agreement (Research Institute employees only)

All benefit eligible staff members must also complete the following documents in the appropriate office of human resources.

  1. Life Insurance Application

  2. Health Care Enrollment Forms (Anthem Health Care) or Waiver form (Choose Waiver form for:  Employee, Retiree, or Grad Assistant)

  3. Dental Insurance Application or Waiver form

  4. TIAA-CREF Group Retirement Annuity Application (University contribution)

  5. Long-Term Disability Application (full-time only)

  6. Short-Term Disability Application
