All faculty and staff members are issued an identification card that enables them to use the various facilities where faculty and staff identification is necessary, i.e. library, door access, etc. A new staff member must obtain an authorization form in the appropriate office of human resources. Identification cards are issued at the One Card Office located in Powerhouse Room 102. Hours of operation are 8:30 to 4:30 Monday through Friday.  For special assistance and additional information call (ext. 92456).

The photos taken and used on your ID card are also automatically uploaded to an internal website located at http://address.udayton.edu.  This site is a directory of phone, fax, e-mail, campus addresses and also provides a tool for password reset and ListServe subscription options.  The site is not available to anyone outside the UD community unless you modify that security setting when you log into the website.   By signing this form, you acknowledge that you are aware that the photo will be used and that it is your responsibility to disable this feature at the above website if you choose not to display your photo on this internal website.

 HOW TO DISABLE OR CHANGE PHOTO AT http://address.udayton.edu

  1. Log in using your Novell user name and password.
  2. To view or edit information about you, select “My Record” under Personal Information on the left side of the screen.
  3. You can edit anything contained in the boxes.  If other information is incorrect, you must contact the appropriate office of human resources to get it changed.
  4. If you want a different image to be used on this site than the one taken at the One Card Office for your ID card, select “Edit Image” under Personal Information on the left side of the screen.
  5. If you want to disable the photo so it cannot be seen by the campus community, select “Security Settings” on the left side of the screen. When the new window appears, select “Hide From Everybody” from the drop down menu next to the word “Image.”  Please note that you will still be able to see the image, but the rest of the UD community will not be able to see it.
