Follow these instruction:

1. Before downloading any form it is recommended to read the following document in order to be familiarized with basic concepts and facts related to Hazardous Waste Disposal. Click Here.
2. Click on the form you need and save the file to disk.
3. Open the file using MS Word 97/2000. or Adobe Acrobat
4. Before printing the document, make sure that the printer uses blank paper. (No letterheads).
5. Print out the document and fill out the form, and send it to Environmental Health and Safety/Risk Management at +2905.
